What is your office’s policy on appointment cancellation? We understand that emergencies happen, and schedules change but if you are unable to make your appointment, please reschedule as soon as possible. Click here to view our Appointment Non-Compliance Policy
Can you call-in my medication if I have to cancel an appointment and will run out prior to the next visit? Many of the medications prescribed are controlled substances, meaning they cannot be called into a pharmacy. Please ensure adequate time between appointments to ensure you do not run out of medication.
Is a drug screen required for all patients? Yes, all new patients will be required to take a urine drug screen. During your care with WK Spine and Pain, intermittent screening will be performed as well.
What kind of paperwork will I need to fill out prior to my appointment? All new patients and established patients that have not been seen in our office in the past 12 months will need to fill out the appropriate new patient paperwork. New patients coming in for an injection-only will complete the New Patient Injection Only paperwork. Established patients scheduled for their regular Follow up appointment will also need to fill out the Follow Up Paperwork prior to EVERY visit.
