Spine and Pain Specialists schedules appointments Monday – Friday. If you would like to schedule an appointment with us, please call our office at (318) 212-7960 to speak with our Referral Coordinator. You should also discuss your interest in our clinic with your primary care physician. Our Referral Coordinator can schedule your first appointment with us at either of our locations.

You will be asked to provide a list of your current medications, as well as medical records pertaining to your pain condition. Once your information is reviewed by a staff member, our Referral Coordinator will call you with information about your appointment.

Required Patient Forms

Save time by printing and completing your new patient information packet before your visit. Other signature paperwork will need your signature but must be competed the day of your appointment in our office.

Fees & Billing

All Co-pays and Deductibles are due at the time of your visit.

We ask that you provide your current insurance information at every visit, to make sure we have your most up to date insurance information

Spine and Pain Specialists accepts most insurance plans. Your insurance coverage is an agreement between you and the insurance provider . We are required to collect all Co-pays and deductibles that apply at the time of service.

We ask that you provide us with a current ID along with your insurance cards. As always the final responsibility of the clinic charges are the patients responsibility. Failure to keep us informed of any changes in coverage that may occur with your current insurance coverage could result in full patient responsibility.
